Job Description 1. Participate in evaluating referrals to assure appropriate admission. A. Obtain permission for delivery of care.

B. Demonstrate understanding and respect for cultural practices. C. Interview Patient / family for purposes of appropriateness for Hospice.

2. Perform psychological assessment of referred patients and their families. A. Interview patient and family for purpose of assessing family structure, dynamics, adjustment to terminal illness, and access to necessary resources.

B. Exhibit knowledge of individual psychosocial development, family structure, and cultural values. C. Exhibit knowledge of medical diagnosis and predicted prognosis.

D. Identify patients needing psychosocial services. E. Educate patient and family about community resources. F. Reassesses patient’s condition and plan of care and re-evaluates and updates as necessary.

3. Formulate and implement social work plan of care in conjunction with interdisciplinary team. A. Meet with interdisciplinary team to discuss patient needs.

B. Demonstrate ability to use clinical skills in counseling patient and family dealing with terminal illness. C. Demonstrate knowledge of community resources and ability to facilitate appropriate referrals.

D. Demonstrate ability to communicate with community agencies in order to provide information to facilitate referral. E.

Assist patient and family to understand and negotiate community resources. F. Demonstrate ability to define community resources to interdisciplinary team to facilitate referral.

G. Incorporate safety, physical, development and psychosocial needs of child, adolescent, adult and / or older adult patient.

H. Incorporate developmental and physiological needs of the child, adolescent, adult and / or older adult in planning and delivery social work part of plan of care.

I. Maintain a caseload of 25 patients (prorated for part time staff). J. Provide care for more than 3 patients daily. 4.

Coordinate psychosocial care to meet the needs and concerns of the patient and family. A. Collaborate with interdisciplinary team to identify unmet patient and family needs and concerns.

B. Update and sign care plans. C. Seek resources to meet needs and concerns. D. Demonstrate knowledge of a variety of social work treatment modalities, i.

e., individual, family, group. E. Anticipate, understand, and meet the needs of the patient / family with empathy and concern demonstrating support and providing help before being asked.

F. Teach and counsel patient / family / caregiver regarding psychosocial care options. 5. Act as a liaison, when needed between patient / family, hospice team and outside resource agencies.

a. Demonstrate knowledge of crisis intervention and emergency community resources. 6. Accurately document all care in an appropriate and timely manner on computer and written forms in accordance with HCN policies and procedures as well as state and federal regulations and JCAHO standards.

A. Develop a written / computerized plan of care for each patient. B. Demonstrate integration of multidisciplinary documentation.

C. Clinical notes and / or progress notes. D. Submits documentation within agency guidelines. E. Documentation is accurate, complete and legible.

F. Chart review scores 85% or more. 7. Serve as a counselor resource to staff and volunteers. A. Demonstrates it through concrete services.

B. Available to meet and dialogue regarding clinical issues. C. Develop and present educational program for volunteers, employees, community members, and / or professional organizations.

8. Demonstrate and participate in clinical supervision (individual and group). A. Discusses concerns with supervisor as needed.

B. Brings psychosocial concerns to social work group supervisor for learning and problem solving. 9. Performs job functions in a fiscally responsible manner consistent with agency’s mission and vision.

A. Utilizes sound time management skills in planning day-to-day activities and fulfilling job responsibilities. B. Demonstrates appropriate and efficient use of supplies and equipment.

C. Consistently meets the productivity standard established by the agency. 10. Demonstrate and promote respect for patient’s rights.

A. Demonstrates awareness of and compliance with Patient’s Bill of Rights. B. Maintains patient and family confidentiality.

C. Assists in making reasonable accommodations for special groups, i.e., hearing, visually, physically, and / or mentally impaired.

11. Incorporates HCN’s commitment to customer service and continuous quality improvement into daily work habits. A . Courteous and considerate in person and on the telephone.

B. Promotes positive co-worker relationships, team work and cooperation both within the department and across departmental lines.

C. Participates in performance improvement data collection and Continuous Quality improvement projects, as assigned, and takes individual responsibility.
