Models, encourages, trains, guides and monitors members in completion of activities necessary to the daily operation of the organization and the attainment of individual members’ goals.
Provide activities for socialization, wellness; community integration, vocational and recreational programs. Assist with any community networking activities.
Adheres to International Center for Clubhouse Development, Commission on Accreditation of Rehabilitation Facilities (CARF) and State of Illinois Department of Human Services standards in all program related activities.
Follow all policies and procedures of the Clubhouse program and Comply with the Midwest Brain Injury Clubhouse Code of Ethics.
PHYSICAL DEMANDS Moderate lifting required during transfer assistance of members and pushing of wheelchairs.
Contact the case managers, Executive Director, caregivers or families, as applicable, within 24 hours of incident or concern.
Follow the procedures for documentation including incident reports and file the appropriate documentation within 24 hours in the member’s chart.
Make any referrals needed in order to meet the individualized member’s goals and assists with locating resources to meet these needs including but not limited to, basic self-
care and related disability issues.
Perform assigned duties in a manner that is safe for the members, co-workers and self, most notably issues of infection control.
For outing safety, ensure that seat belts are utilized by all members, staff and volunteers and that complete supervision is provided including travel (i.e. cabs or buses).
Basic knowledge of disabilities and interventions as it applies to brain injury and stroke
Basic Microsoft Office skills
Current enrolled in a health or human service related field or a high school education with work experience in the field of disability social services.
CNA / Caregiver work experience is a plus
Must have completed NR224, NR226, NR302, & NR 304
