KORT in New Albany, KY is seeking a per diem (PRN) home health social worker. Hours are not guaranteed with per diem (as needed basis) but it is a GREAT opportunity to join a local leader in your community.

KORT is Kentucky's premier provider of outpatient physical and occupational therapy specializing in orthopedics, sports and industrial medicine.

Since 1987, KORT has provided clinical and customer service excellence to patients in our clinics, at employer sites, and with high school, college, and professional sports teams.

  • In addition, KORT Home Health also provides home health services in Southern Indiana including : post-operative physical therapy;
  • fall prevention; vestibular rehabilitation; wound care; and more.

    The Social Worker is a member of the Multidisciplinary health care team whose role is to empower the patient to utilize medical care in order to reach their optimal level of health and quality of life by providing emotional and psychosocial support and access to basic needs.

    This may involve assessment, discharge planning, counseling, support groups, patient advocacy, community resources, ethics committee involvement, or facilitating communication between patient, family and health care team.

    Employees in this position will :

    1. Assesses psychosocial needs of patients and their families to establish appropriate plan of care and discharge planning. Measures :

    a. Interview and assess patients / families to identify psychosocial and / or resource needs within 24 hours of referral.

    b. High risk screening factors are used to initiate discharge planning early in admission when feasible.

    c. Document psychosocial assessment in patient chart according to policy.

    d. Establish plan of care based on assessment.

    e. Develop discharge plan options for patient / family choice.

    f. Communicate patient and family needs to pertinent members of the health care team.

    g. Establish goals and discharge plan with patient and family, respecting the patient’s and family’s choices and values.

    2. Provides individual and group psychosocial support when indicated to alleviate anxieties and fears related to illness, and adjustment to mental and physical changes. Measures :

    a. Identifies need for emotional support by an assessment and / or referral.

    b. Provides individual emotional support utilizing appropriate intervention skills.

    c. Promotes, develops, and facilitates support group utilizing group work skills.

    d. Provide educational information as requested.

    e. Identify need for referral to mental health professional and facilitate referral to community resource.

    f. Refer to consulting psychiatrist as appropriate.

    3. Demonstrates comprehensive knowledge of community resources and case management through the development of collaborative relationships with community resources. Measures :

    a. Develops and maintains knowledge base for community resources and third party payers.

    b. Develops and maintains open communication and relationship with vendors, resources, and facilities.

    c. Understand medical terms and treatments for age / disease specific caseload to facilitate appropriate discharge planning.

    d. Coordinates social work plan of care and discharge plans with health care team, patient, and family, to ensure continuity of care and protect patient rights.

    4. Demonstrates professional social work skills, professionalism, and ethics in the delivery of quality patient care. Measure :

    a. Upholds the professional Social Work standards, and practices code of ethics.

    b. Maintains confidentiality.

    c. Demonstrates respect for all individuals.

    d. Accepts responsibility and accountability.

    e. Departmental issues are treated with respect, confidentiality, and integrity.

    f. Maintains licensure and CEU’s per Social Work Licensing Board. Provides Human Resources Department and Supervisor current social work license.

    g. Practices within licensure scope of practice.

    h. Observes and assesses self, is willing to receive feedback, and is able to incorporate information into practice.

    i. Meets deadlines and prioritizes tasks and caseload.

    5. Performs administrative tasks and duties related to services provided by the Social Services Department. Measures :

    a. Is timely and accurate, and follows departmental policies for documentation and case files.

    b. Adheres to the departmental standards regarding use of referral systems, discharge planning, and timeliness.

    c. Coordinates and / or participates in the development of projects related to department as assigned.

    d. Maintains knowledge of current departmental and hospital policies as demonstrated in the appropriate performance of duties.

    e. Attends staff meetings or reviews staff meeting minutes and incorporates information into practice.

    f. Follows departmental policies and standards regarding personal leave, time recording and on-call-rotations for department coverage.

    g. Participates in departmental committees as appropriate.

    h. Participates in qualify improvement initiatives in the department and hospital-wide.

    i. Participates in educational opportunities for departmental development such as Competency Based Staff Development.

    6. Perform other duties as requested.

    Minimum Skills, Knowledge & Abilities (including licensure, certification and other job-related credentials)

    1. Bachelor’s Degree in Social Work required.

    2. Licensed Bachelor Social Worker (LBSW) required and Licensed Clinical Social Worker (LCSW) or Licensed Master Social Worker (LMSW) preferred

    3. 1 Year of previous experience required.

    4. Certified BLS completion required; for new hires, see CPR Requirement Guidelines for New Hires.

    5. Valid Drivers License with good driving record
