We have a wonderful opportunity for a Social Worker in our Foster Care and Adoption Services Program. The Social Worker has the responsibilities of determining the needs of the foster child and the foster family and either providing the service or seeing to it that it is provided.
Also supports all agency adoptions.
ESSENTIAL DUTIES AND RESPONSIBILITIES :
Provide case management by being responsible for maintaining the compliant contents of the child’s file, creating and ensuring implementation of the treatment plan, monitoring the child’s activities through home visits, contact with the foster parents, and collaborating with the child’s teacher(s)
Complete or obtain assessments (CANS) within first 60 days of placement
Complete Needs & Service Plan with input from Child & Family Team within first 30 days of placement
Complete quarterly updates of Needs and Service Plan objectives and send copy to Placement Worker
Complete Discharge summary within 7 days of child / youth / NMD’s discharge
Be part of transition plan with Child & Family Team pre admission and post discharge
Serve as the primary contact for the placing county and, with them, advocate for the client’s needs, coordinate treatment team meeting, actively plan for the child’s permanency and discharge from the agency programs
Provide crisis intervention and solve practical problems and deal with a variety of unpredictable situations where only limited standardization exists
Supervise visits between children and birth family members
Attend Child & Family Team meetings
Ensure that outcomes and metrics are met for families and children towards permanency and lifelong connections
Support Resource family in ensuring that child feels safe in school environment, that foster family acts in alignment with reasonable and prudent parent standard
Ensure that child is enrolled and participating in age appropriate enrichment activities
Ensure that resource family is providing culturally relevant services and supports to child / youth / NMD including LGBTQ / SOGIE and CSEC
Conduct monthly facility inspections for resource family home and document corrections and compliance in maintaining licensing requirements
Complete training to meet RFA core competency requirements as outlined in training plan
Provide training, support and education to the foster parents
Submit reports to the Court and social services as required
Conduct initial interviews to screen potential foster families for appropriate placement of children
Conduct home studies and home study updates of foster families in a timely manner
Complete adoption finalization paperwork in a timely manner
Work cooperatively with staff and volunteers
Participate in group processes and activities such as staff meetings, trainings, and conferences
Provide transportation to clients when necessary
Complete special projects as required
Ensure that any BWC vehicles used are kept tidy and that any maintenance or safety issues are reported to the Facilities Manager immediately.
Ensure that site is neat and tidy and report all maintenance or safety issues to manger or Facilities Manager immediately.
Participate in agency Program Quality Improvement activities and initiatives
Participate in agency Safety initiatives and proactively report safety issues to management immediately
Regular attendance is a requirement of the position.
Other duties may be assigned.
Master’s Degree in Social Work required. LCSW highly preferred. Social worker will possess working knowledge of child welfare system and knowledge of trauma, separation and loss.
An understanding of Community Care Licensing and the regulations controlling Foster Care children and families. Experience working with culturally diverse youth required.
Valid CA drivers license with access to a personal vehicle with current Ins. Fingerprinting, TB test & CPR / First Aid required.
