Description RN Clinical Care CoordinatorThe Ambulatory RN Clinical Care Coordinator provides for the delivery of patient care utilizing the Nursing Process within the framework of the Nurse Practice Act, ANA Code for nurses and Standards for Nursing Practice. The Ambulatory RN Clinical Care Coordinator impacts patient care by improving clinical nursing practice through consultation, role modeling, and educational programs. The Ambulatory RN Clinical Care Coordinator provides nursing care and education for an assigned patient population and facilitates the Strong Start Centering program to assure success of Shared Medical Appointments. SHIFTPrimary Clinic hours Monday Friday 8:00 AM - 5:00 PMWork schedule must be flexible to accomidate patient needs. 40 hours per week. Role has potential to become full time in future. WHAT YOU’LL DO1. Cares for patients using the Nursing Process 2. Provides care based on physical, psycho/social, educational, safety and related criteria, appropriate to the ages of patients/customers served in assigned area. 3. Communicates pertinent information to patients, family, staff 4. Adapts to change in a positive constructive manner. 5. Participates in activities for professional development and maintains required clinical knowledge, technical skills, training, and credentials. 6. Plans, initiates and coordinates care and education for patient / family and staff.
